A bronchial Adenoma is a type of tumor in the windpipe (trachea) or large airways of the lung (bronchi) that usually blocks the airway. Although the term bronchial Adenoma was once used to refer to noncancerous (benign) airway tumors, these tumors (with the exception of mucous gland adenomas), are now known to spread to other areas of the body.

Of 48 patients having clinical hyperthyroidism caused by proved hyperfunctioning Adenoma, as determined by I scintigrams 25 had normal 24-hr thyroidal I/ sup 131/ uptake values. The I/ sup 131/ scintigrams of the 46 patients revealed that only 1 hyperfunctioning Adenoma of the thyroid was present in each goiter. No significant I/ sup 131/ uptake was observed in the normal thyroidal tissue outside of the Adenoma. The results of therapy with an initial large dose I/ sup 131/ , delivering from 15 to 30 mC (average 21.4 mC) to the Adenoma, showed that 76% of the patients were euthyroid within 2 months, and all 21 patients within 4 months, after treatment. In contrast, when a moderate initial dose of I/ sup 131/ delivering 6 to 14 mC (average 10.1 mC) to the Adenoma was given to 38 patients with toxic nodular goiter (presumed hyperfunctioning Adenoma) with hyperthyroidism, only 16% were euthyroid within 2 months and 53% within 4 months. In each of the 21 patients the hyperfunctioning Adenoma decreased in size but did not disappear after I/ sup 131/ treatment. No significant clinical exacerbation of hyperthyroidism occurred. No significant early or late radiation effect has yet been observed in any of the patients treated with I/ sup 131/ , and in none of the treated patients has hypothyroidism developed. (TCO)

A majority of the participants may have been considered at a higher risk of colorectal Adenoma. Evidence of this bias can be seen the fact that controls were younger and were more likely to have had a history of colorectal cancer in their family than cases.

DESCRIPTION (provided by applicant): Defining mediators and mechanisms which favor the early stages of the colon cancer such as formation of Adenoma, is integral to improved surveillance and prevention of colon cancer. Our recent studies demonstrate strong correlations between reduced apoptosis in the normal intestinal mucosa and risk of precancerous adenomatous lesions in humans. Preliminary and published evidence indicates elevated plasma insulin and increased levels or bioavailability of circulating or locally expressed insulin-like growth factors (IGF-I or IGF-II) as significant risk factors for reduced apoptosis and Adenoma in the colon. Common mediators of insulin and IGF action on apoptosis or Adenoma growth may represent particularly powerful biomarkers of Adenoma risk. Insulin receptor substrate 1 (IRS-I) is a common mediator of the anti-apoptotic actions of IGF-I, IGF-II and insulin. Preliminary data in mouse models demonstrate that IRS-1 is required for anti-apoptotic actions of IGF-I in the normal intestine in vivo and that loss of just 1 copy of the IRS-1 gene increases apoptosis and reduces Adenoma formation in the APC/ Min-+model of intestinal polyposis. Recent studies link polymorphisms in the human IRS-1 gene to colon cancer risk. Together, this data supports our central hypothesis that levels of expressed IRS-1 in normal human colonic mucosa predict the levels of apoptosis and Adenoma risk by mediating the anti-apoptotic actions of insulin and the IGFs. Our studies will use existing samples from a large case-control study of colorectal adenomas to test our hypothesis. Data on apoptosis, plasma insulin, IGFs and plasma or colonic levels of insulin-like growth factor binding proteins (IGFBPs) have already been collected in this same study population. Aim 1: will test the hypothesis that a Gly972Arg polymorphisms in the IRS-I gene, and/ or levels of expressed IRS-1 mRNA in the normal colonic mucosa predict Adenoma risk and levels of apoptosis.
