Patients with severe Action Tremor have uncontrollable, relatively rapid oscillatory motion super-imposed on otherwise useable slower voluntary motor activity. Because a mechanical damper produces an opposing force proportional to velocity, applying damping loads to tremorous limbs should attenuate the (high-velocity) Tremor component of movement while permitting the slower purposeful portion to proceed relatively unopposed. In this study, the effect of upper extremity damping in three degrees of freedom was examined in 10 patients with cerebellar Action Tremor due to multiple sclerosis or traumatic brain injury. Variable amounts of damping were applied by prototype energy-dissipating orthoses which generated resistive viscous loads by means of computer-controlled magnetic particle brakes. All patients experienced statistically and functionally significant Tremor reduction with the application of damping.

Tremor in multiple sclerosis The predominance of Action tremors (postural and intention) in patients with MS point to the cerebellum and its connections as the most likely source of Tremor production, whereas the rarity of rest Tremor argues against an involvement of the basal ganglia. The common occurrence of bilateral Tremor might indicate that damage to the cerebellum and its connections is often multifocal.

A useful way to understand and describe tremors is to define them according to the following types. Resting or static Tremor occurs when the muscle is relaxed and the limb is fully supported against gravity, such as when the hands are lying on the
lap. It may be seen as a shaking of the limb, even when the person is at rest. This type of Tremor is often seen in patients
with Parkinson s disease. An Action Tremor occurs during any type of movement of an affected body part. There are several subclassifications of Action Tremor. Postural Tremor occurs when the person maintains a position against gravity, such as holding the arms outstretched. Kinetic (or intention ) Tremor occurs during purposeful voluntary movement, such as touching a finger to one s nose during a medical exam. Task-specific Tremor appears when performing highly skilled, goal-oriented tasks such as handwriting or speaking. Isometric Tremor occurs during a voluntary muscle contraction that is not accompanied by any movement.

Acute Flaccid Paralysis Syndrome Associated with West Nile Virus Infection --- Mississippi and Louisiana, July--August 2002 Case 3. In July 2002, a man aged 56 years from Louisiana with a history of hypertension and coronary artery disease was hospitalized with a 4-day history of fever, vomiting, and painless asymmetric leg weakness. On examination, the patient had a flaccid areflexic right leg and a weak, hyporeflexic left leg; strength and reflexes in the arms were normal. The patient had decreased sensation in a stocking-and-glove distribution and a coarse upper extremity Action Tremor. A lumbar puncture revealed a CSF pleocytosis ( Table ). He was admitted with a diagnosis of postviral demyelination syndrome and treated with antimicrobial medication, IVIG, and dexamethasone. MRI of the spine revealed mild cervical spinal stenosis and homogeneous enhancement of the cauda equina consistent with meningitis. EMG/ NCS were indicative of a severe asymmetric process affecting anterior horn cells and/ or their axons. IgM and neutralizing antibody test results confirmed an acute WNV infection.
